💼 About This Role

You'll join the Overdose Data to Action program to provide education, outreach, and linkage to care for individuals with substance use disorders. You'll conduct community outreach and build partnerships with service providers to increase access to resources. This role supports a CDC Foundation initiative in Southern Nevada.

🎯 What You'll Do

• Provide outreach and care coordination for individuals with substance use disorder.
• Develop networks with external stakeholders and service providers.
• Assist with assessing and managing referral processes.
• Use overdose data to identify hotspots and conduct targeted outreach.

📋 Requirements

• Minimum 3 years of experience in linkage to care or case management.
• Knowledge of overdose prevention and substance use disorder treatment.
• Ability to coordinate with diverse stakeholders.
• Proficiency in Microsoft Excel, Word, PowerPoint, Teams and Zoom.

✨ Nice to Have

• Bachelor's degree in public health or related field.
• Experience with trauma-informed and strengths-based approaches.
• Knowledge of community resources.
