💼 About This Role

You'll provide hands-on patient care in home health settings across Napa and American Canyon. You'll work under a Case Manager to implement care plans, perform skilled procedures, and educate patients. This entry-level role builds foundational skills for case management advancement.

🎯 What You'll Do

• Perform hands-on procedures like wound care, PICC lines, and trach care
• Implement and document care plans within 24-48 hours
• Educate patients and families on self-care techniques
• Coordinate with interdisciplinary team and attend case conferences

📋 Requirements

• One year of recent nursing experience
• Current CA Registered Nurse License
• Current Basic Life Support card
• Valid CA Driver's License with acceptable driving record
