Description

You will work on a per-diem basis providing home health visits to adult and geriatric patients in East Baltimore County. As part of a multidisciplinary team, you will conduct clinical social evaluations, connect clients with community resources, and advocate for their needs to help them remain safely at home.

Requirements

• Master's in Social Work from CSWE-accredited program
• Licensed by Maryland Board of Social Work Examiners at LMSW or LISW level
• At least 2 years of community-based experience
• Ability to travel to cases
• Solid computer skills, EMR experience preferred

Responsibilities

• Make home visits to clients in designated territories
• Provide clinical social evaluations and plan interventions
• Identify appropriate community resource referrals
• Act as advocate for client and family to navigate community systems
• Participate in developing total plan of care with multidisciplinary team
