Requirements

• Board-Certified in Family or Internal Medicine
• 3+ years of post-residency work experience
• Minimum 2+ years in an in-person primary care setting
• Experience with virtual visits required
• Active OR state medical license; IMLC preferred

Responsibilities

• Provide high-quality preventative, acute, and chronic care in a fully remote, virtual setting
• Manage a dedicated panel of patients as their primary care physician
• Work on population health initiatives for risk-attributed patients
• Support patients immediately post-discharge to reduce hospital readmissions
• Practice continuous self-evaluation to adhere to clinical guidelines
