💼 About This Role

You'll provide rehabilitative care to adult and geriatric patients in home health visits throughout Lackawanna County, as part of a multi-disciplinary team. Your work enables clients to remain safely at home.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations of patients' functional status
• Develop individualized intervention plans to achieve favorable outcomes
• Educate patients and family members on rehabilitative care

📋 Requirements

• Current Pennsylvania Occupational Therapist license
• Minimum of one year of recent work experience as an Occupational Therapist
• Graduation from an accredited occupational therapy program
