💼 About This Role

You'll provide skilled home health physical therapy, evaluating patients and developing care plans to restore function and promote independence. You'll coordinate with physicians and PTAs to ensure quality, goal-directed care in the Placerville area. This role offers autonomy with flexible scheduling and targeted territories.

🎯 What You'll Do

• Perform home evaluations and develop care plans
• Help patients with mobility and regain independence
• Communicate and coordinate with physicians
• Evaluate and report patients' progress to MD

📋 Requirements

• Current unrestricted CA Physical Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
