💼 About This Role

You'll provide skilled home health physical therapy to patients in Solano County, helping them regain independence after illness or injury. You'll develop care plans, coordinate with physicians, and supervise PTAs to ensure quality outcomes. This role offers autonomy with flexible scheduling and targeted zip-code-based assignments to minimize driving.

🎯 What You'll Do

• Perform in-home patient evaluations and develop care plans.
• Help patients with mobility and regain independence.
• Coordinate with physicians and report patient progress.
• Supervise Physical Therapy Assistants as needed.

📋 Requirements

• Current unrestricted CA Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
