Description

You will provide home health services to adult and geriatric clients on a per visit basis in Scotland, Richmond, and Robeson counties, working as part of a multi-disciplinary team to offer rehabilitative care and skilled nursing so clients can remain at home.

Requirements

• Prior clinical experience as a licensed Physical Therapist required
• Home health/OASIS documentation experience required
• 2-3 days/week availability required

Responsibilities

• Make home visits to clients in designated geographic territories
• Provide rehabilitative care and skilled nursing to adult and geriatric clients
