Description

As a hospice nurse (RN), you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, enabling them to remain at home. You will make home visits, perform assessments, administer medications, and provide comfort care.

Requirements

• Current Registered Nurse (RN) license in PA
• Minimum 1 year of recent clinical (medical/surgical) experience
• Prior home care or hospice experience strongly preferred
• Solid computer skills; prior EMR experience preferred
• Ability to travel to cases as assigned

Responsibilities

• Make home visits to clients in designated geographic territories
• Administer medication, wound care, treatments, procedures, and comfort care
• Monitor clients' conditions and report changes to clinical manager
• Follow up on and document doctors' orders
• Perform client assessments and occasional on-call after hours/weekends
