Description

You will provide rehabilitative care and skilled nursing to adults and geriatric patients in their homes, evaluating functional status and developing individualized intervention plans to promote independent living.

Requirements

• Current DE Occupational Therapist license
• Graduation from an accredited occupational therapy program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills
• Solid computer skills, EMR experience preferred

Responsibilities

• Perform thorough evaluations of patients' functional status, including ADL and IADL performance
• Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans
• Evaluate home environment for hazards and identify equipment needs
• Educate patients and family members in rehabilitative care and activities
• Accurately document observations, interventions, and evaluations using a touch pad tablet
