💼 About This Role

You'll serve as a vital clinical resource for cancer members, providing triage, support, and education via phone and email. You'll conduct comprehensive clinical assessments and manage symptom management and longitudinal care coordination. This role focuses on empathetic, human-centered support in a tech-native value-based care company.

🎯 What You'll Do

• Conduct comprehensive clinical assessments for cancer patients.
• Provide triage, symptom management, and care coordination.
• Facilitate end-of-life care discussions.
• Collaborate with care team and external providers.

📋 Requirements

• Active unrestricted compact Registered Nurse (RN) license.
• Degree in Nursing (ADN or BSN).
• At least 5 years of nursing experience including 3 years in solid tumor oncology.
• Oncology-related certification (OCN, AOCN, AOCNS, or CCM) within 2 years.

• Compact and single-state licensure in New York and/or Illinois.
