💼 About This Role

You'll provide direct psychosocial support to participants as part of an interdisciplinary team. You'll coordinate community services and manage care plan reviews to help seniors achieve their full potential. This role offers work-life balance with generous PTO and holidays.

🎯 What You'll Do

• Establish rapport with participants and families to provide psychosocial support.
• Deliver and document social work interventions within scope of practice.
• Coordinate services such as housing, DMV needs, and utility bills.
• Conduct home, hospital, and skilled nursing visits as necessary.

📋 Requirements

• High school diploma or equivalency.
• Reliable means of transportation.
• Must obtain CPR and First Aid certification within 180 days of hire.

• Associate degree in social work, human services, or relevant field.
• Previous experience assisting people with behavioral health and substance abuse issues.
