💼 About This Role

You'll provide education, outreach, and linkage to care for individuals with opioid use disorder. You'll conduct community outreach and build partnerships to connect people to substance use disorder services. This role supports the CDC Foundation's OD2A program.

🎯 What You'll Do

• Conduct outreach to engage individuals in care.
• Provide referrals and care coordination for substance use disorder.
• Develop networks with community partners and stakeholders.
• Assess referral processes and address patient challenges.

📋 Requirements

• 3+ years of experience in linkage to care or case management.
• Knowledge of overdose prevention and substance use disorder treatment.
• Experience with trauma-informed and strengths-based approaches.
• Proficiency in Microsoft Office (Excel, Word, PowerPoint, Teams, Zoom).

✨ Nice to Have

• Bachelor's degree in public health or social sciences.
• Knowledge of community resources.
• Experience working in a virtual environment.
