Description

You will provide 1:1 care to clients in their homes throughout Monroe County, PA, following a designated care plan and performing duties such as medication administration, wound care, and monitoring conditions. This role offers flexible scheduling and the opportunity to make a meaningful difference in patients' lives.

Requirements

• Current RN license in Pennsylvania
• Minimum 1 year recent clinical (medical/surgical) experience
• Graduation from accredited nursing program
• Ability to travel to cases
• Strong interpersonal and computer skills

Responsibilities

• Follow designated care plan in accordance with patient's needs
• Perform assigned duties including medication administration, wound care, and procedures
• Monitor clients' conditions and report changes to manager
• Perform client assessments and document care using touch pad tablet
• Case management and coordination
