💼 About This Role

You'll build relationships with hospital contacts to educate them about BAYADA's hospice services. You'll manage referral intake and coordinate safe discharge. This role combines clinical expertise with sales to drive growth.

🎯 What You'll Do

• Develop referral relationships with hospital and facility contacts.
• Educate patients and staff on hospice eligibility and benefits.
• Coordinate discharge and initiate GIP level of care.
• Complete evaluations and document GIP eligibility daily.

📋 Requirements

• Active Registered Nurse license in state of practice.
• Graduate of an accredited nursing program.
• 2 years experience in healthcare, preferably in hospice or liaison role.
• Demonstrated record of building and maintaining referral relationships.

✨ Nice to Have

• Previous discharge planning experience.
• Knowledge of Medicare COPs and program requirements.
