Description
You will provide telemedicine-based comprehensive care to cancer survivors, including assessments, personalized treatment planning, and coordination with a multidisciplinary team. This role offers the opportunity to shape survivorship care programs and improve patient outcomes.
Requirements
- 3-5+ years advanced practice experience in oncology
- Preferred experience in cancer survivorship care or supportive care management
- Master's degree in nursing
- Current NP certification in appropriate specialty for adult patients
- Willingness to obtain licensure as a registered NP in specified states
Responsibilities
- Conduct comprehensive assessments of cancer survivors via telemedicine
- Provide evidence-based guidance and personalized treatment planning
- Develop and update survivorship care plans in collaboration with patients
- Coordinate multidisciplinary care with oncologists, PCPs, and specialists
- Work with clinical leadership to build and enhance survivorship care programs
