🎯 What You'll Do

• Conduct home visits and community outreach to high-complexity patients
• Assess patient needs and help set health goals

📋 Requirements

• 2+ years experience as a Community Health Worker, Peer Support Specialist, or similar role
• High school diploma or GED
• Valid driver's license and auto liability insurance
• Experience in care coordination for chronic conditions or behavioral health

• Prior home care or Enhanced Care Management experience
