Description

You will make home visits to provide occupational therapy to adult and geriatric patients, evaluating functional status, developing care plans, and educating patients and families. You'll collaborate with a multidisciplinary team to help clients remain at home safely.

Requirements

• Current VA Occupational Therapist license.
• Minimum 1 year recent work experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior experience with EMR preferred.

Responsibilities

• Perform thorough evaluations of patients' functional status, including ADL and IADL performance.
• Develop individualized intervention plans to achieve favorable outcomes.
• Evaluate home environment for hazards and recommend equipment adaptations.
• Educate patients and families on rehabilitative care and independent living activities.
• Document patient care observations and interventions using electronic medical records.
