Description

You will provide rehabilitative physical therapy to home health patients, including assessment and treatment of mobility, gait, and transfer capabilities. Work as part of an interdisciplinary team to deliver coordinated care. Enjoy flexible scheduling with PRN shifts and competitive pay.

Requirements

• Bachelor's or Master's degree in Physical Therapy
• Minimum 2 years of physical therapist experience; home health preferred
• Current Ohio Physical Therapist license (or application in process)
• Valid Ohio driver's license and reliable transportation
• Current CPR certification

Responsibilities

• Provide physical therapy services per plan of care, including assessment and treatment of muscle strength, mobility, gait, ROM, and transfer capabilities
• Instruct patients, families, and caregivers in use of therapeutic appliances and therapy programs
• Supervise Physical Therapist Assistant as necessary
• Confer with attending physician and home care team to coordinate comprehensive care
• Prepare and submit clinical and progress notes in compliance with guidelines
