You'll conduct in-home care assessments and coordinate with the interdisciplinary team to help seniors thrive in their homes. 🎯 What You'll Do

• Coordinate incoming calls regarding physician orders and referrals
• Communicate with participants and coordinate schedule changes
• Collaborate with home care services to track delivery and hours
• Assist with staffing and scheduling activities

📋 Requirements

• Healthcare/medical licensure or equivalency
• At least three years of professional experience
• Minimum of three years of case management or nursing experience with frail/elderly population

• Bachelor's degree
• Nursing knowledge for treating frail elderly
• Care planning and management experience in home care
