Description

You will provide rehabilitative care and skilled nursing to adult and geriatric patients in senior living facilities, performing evaluations, developing intervention plans, and educating patients and families to promote independent living.

Requirements

• Current MD Occupational Therapist license
• Minimum one year of recent work experience as an OT
• Graduation from an accredited occupational therapy program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations of patients' functional status
• Develop individualized intervention plans to achieve favorable outcomes
• Evaluate home environment for hazards and identify equipment needs
• Document observations and interventions using a touch pad tablet
