Description

As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team providing rehabilitative care and skilled nursing to clients, allowing them to remain at home. You will perform diagnostic tests, develop individualized care plans, educate clients and families, and document care using a state-of-the-art touch pad tablet.

Requirements

• Current Maryland Physical Therapist license
• One year prior clinical experience as a licensed Physical Therapist
• Current home health and Oasis experience (for per diem role)

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, strength, balance, etc.)
• Implement wound prevention and repair interventions
• Develop and implement individualized care plans including therapeutic exercises
• Educate clients and families on rehabilitative care for independent living
