Description

You will provide rehabilitative care and skilled nursing to adult and geriatric patients in their homes throughout Central and Upper Bucks County. As a key member of a multi-disciplinary team, you will perform evaluations, develop treatment plans, and educate patients and families to support independent living.

Requirements

• Current license as a Speech Language Pathologist in PA
• Certificate of Clinical Competence in Speech from ASHA or equivalent educational and work experience
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform evaluations to assess and diagnose speech, language, communication, or swallowing difficulties
• Develop treatment plans based on evaluations and physician's orders
• Educate patients and family members in rehabilitative care and activities
• Document observations, interventions, and evaluations using a touch pad tablet
