Description

You will lead and inspire a team of field staff to provide exceptional home health care to adult and geriatric clients. You'll oversee clinical documentation, coordinate client care, and act as an internal case manager and client advocate.

Requirements

• 2+ years RN experience in Home Health
• Oasis experience required
• 2+ years in a leadership/supervisory role
• Excellent organization and communication skills
• Strong knowledge of home health policies and procedures

Responsibilities

• Lead, inspire, and motivate field staff
• Review clinical documentation for consistency and accuracy
• Assign appropriate clinicians to each client
• Develop and update individualized care plans
• Coordinate client care and act as case manager
