Description

You will conduct in-home assessments, deliver personalized home modification recommendations, and educate clients on fall prevention. Collaborate with the clinical team to support older adults and high-needs populations in living safely at home.

Requirements

• Degree in Occupational Therapy from accredited program
• Active, unencumbered OT license
• 2+ years clinical experience
• Reliable transportation and valid driver's license
• Proficiency with technology and digital documentation

Responsibilities
