Description

You will make home visits to adult and geriatric patients in West and South Philadelphia, performing evaluations and providing therapeutic interventions for speech, language, communication, or swallowing difficulties. You will develop treatment plans, educate patients and families, and document care using a touch pad tablet as part of a multidisciplinary team.

Requirements

• Current license as a Speech Language Pathologist in Pennsylvania.
• Certificate of Clinical Competence from ASHA or equivalent educational/work experience.
• Solid computer skills; prior experience with electronic medical records preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform evaluations to assess speech, language, communication, or swallowing difficulties.
• Develop a treatment plan based on evaluation and physician's orders.
• Revise the speech therapy care plan and participate in multidisciplinary care plan.
• Educate patients, family, or representatives in rehabilitative care and activities.
