💼 About This Role

You'll be Waymark's frontline presence in the community, connecting patients with social support and resources as part of a multidisciplinary care team. Your work will directly improve health outcomes for Medicaid patients by addressing social determinants like homelessness and hunger. This role offers the chance to build community partnerships and make a tangible difference in underserved populations.

🎯 What You'll Do

• Conduct needs assessments and help patients set health goals.
• Connect patients with community resources like food and housing.
• Manage relationships with healthcare providers and care team.
• Document patient encounters in technology systems.
• Attend weekly care team huddles.

📋 Requirements

• Highly organized and self-motivated to work independently.
• Ability to work with a diverse community empathetically.
• Computer literate with experience using technology.
• Reside within commutable distance of Aurora, Illinois.
• Available to work weekends and late shifts.
