💼 About This Role

You'll perform home health visits for adult and geriatric clients in northeastern Cuyahoga County. You'll develop and implement individualized care plans and document using a state-of-the-art touch pad tablet. BAYADA offers a $10,000 sign-on bonus and multiple career advancement tracks.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility, strength, and balance.
• Develop and implement individualized care plans including therapeutic exercises.
• Educate clients and family members on rehabilitative care and activities.

📋 Requirements

• Current OH Physical Therapist license.
• Minimum one year of recent work experience as a Physical Therapist.
• Graduation from an APTA-approved program.
• Solid computer skills; experience with EMR preferred.
