Description

You will provide outpatient home health visits to adult and geriatric clients in the Coatesville area, focusing on one-on-one patient care and quality over quantity. You will perform diagnostic tests, develop care plans, educate clients, and document using a touch pad tablet.

Requirements

• Current PA Physical Therapist license
• Minimum one year of recent work experience as Physical Therapist
• Graduation from APTA or CAAHEP accredited program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem solving skills

Responsibilities

• Make outpatient visits and perform diagnostic tests and measurements
• Develop and implement individualized care plans including manual therapeutic exercises, gait training, and balance
• Continually assess and revise the PT care plan; participate in multidisciplinary care plan
• Educate clients and family members in rehabilitative care
• Document observations, interventions, and evaluations using touch pad tablet
