Description

You will plan, organize, and direct hospice care for patients and families in their homes, focusing on whole-person support: expert medical care, pain management, and emotional/spiritual assistance. You'll conduct assessments, develop care plans, and coordinate with the interdisciplinary team to ensure compassionate, dignified end-of-life care.

Requirements

• Registered Nurse license
• Experience in nursing with emphasis on community health education
• Ability to complete physical assessments and history
• Knowledge of palliative nursing procedures
• Strong communication skills for patient/family education

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patients and families
• Develop and revise individualized care plans based on nursing diagnosis
• Administer medications and treatments as prescribed
• Provide health teaching and counseling to patients and families
• Coordinate care with physicians and interdisciplinary team
