Description

As a home care Registered Nurse, you will be an integral member of a multi-disciplinary health care team providing rehabilitative care and skilled nursing to adult patients in their homes. You'll follow care plans, perform assessments, administer medications, and document care using a touch pad tablet, helping patients remain at home.

Requirements

• Current Registered Nurse license in Delaware
• Minimum one year recent clinical (medical/surgical) experience
• Graduation from accredited nursing program
• Ability to travel to cases as assigned
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Follow designated care plan in accordance with patient needs
• Make home visits to clients in designated territories
• Administer medication, wound care, treatments, and procedures
• Monitor clients' conditions and report changes to management
• Perform client assessments and accurately document observations
