Description

As a hospice nurse (RN), you will provide palliative care to clients in their homes, enabling them to receive medical care while remaining in a familiar environment. You will perform assessments, administer medications and treatments, and collaborate with a multi-disciplinary team to ensure comfort and quality of life. Requirements

• Current RN license in VT and NH
• Minimum one year of recent clinical (medical/surgical) experience
• Solid computer skills
• Ability to travel to cases
• Prior home care or hospice experience preferred but not required

Responsibilities

• Make home visits to clients in designated geographic territories
• Administer medication, wound care, treatments, procedures, and comfort care
• Monitor clients' conditions and report changes to Clinical or Client Services Manager
• Execute and document doctors' orders
• Perform client assessments as necessary
