Description

As a home care Physical Therapist, you will perform home visits for adult and geriatric clients in Windham County, Vermont. You'll be an integral member of a multi-disciplinary team, providing rehabilitative care that helps clients remain at home. You'll use a state-of-the-art touch pad tablet for documentation.

Requirements

• One year prior clinical experience as a licensed Physical Therapist
• Valid Physical Therapist license
• Ability to perform home health visits

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, range of motion, strength, balance)
• Implement wound prevention and repair interventions using evidence-based dressings
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Educate clients and families on rehabilitative care and independent living activities
