Description

You will provide case management and skilled nursing care to patients in their homes, conducting assessments, developing care plans, administering treatments, and coordinating with physicians and families. The role requires travel throughout the greater Lebanon, Ohio area and includes on-call responsibilities.

Requirements

• Valid Ohio RN license and CPR certification
• Minimum 1 year clinical nursing experience
• Valid driver's license, auto insurance, and clean driving record
• Bachelor's degree in Nursing preferred
• Home Health or Hospice experience preferred

Responsibilities

• Complete initial patient assessments and home care needs
• Initiate and revise plan of care based on patient status
• Administer medications and treatments as prescribed
• Provide health care instructions to patient and family
• Coordinate care with physicians and community health workers
