💼 About This Role

You'll perform home health visits for adult and geriatric patients in Central and Upper Bucks County. As part of a multi-disciplinary team, you'll provide rehabilitative care that enables clients to remain at home.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories
• Perform evaluations and provide therapeutic interventions for speech/swallowing difficulties
• Develop and revise treatment plans based on evaluations and physician's orders
• Educate patients and families on rehabilitative care activities
• Document observations and interventions using a touch pad tablet

📋 Requirements

• Current license as a Speech Language Pathologist in PA
• Certificate of Clinical Competence in Speech from ASHA or equivalent
• Solid computer skills

• Prior home care experience as a Speech Language Pathologist
• Prior experience with electronic medical records (EMR)
