Description

You will provide bedside care to hospice patients, support families and caregivers, and collaborate with the RN Case Manager. This PRN (as-needed) role involves on-call duties, continuous care, and routine visits in a community-based, mission-focused ministry.

Requirements

• Graduate of approved nursing program
• Valid Ohio LPN license
• CPR certification
• 2-4 years direct patient care in hospice or home care
• Valid driver's license and auto insurance in Ohio

Responsibilities
