💼 About This Role

You'll utilize your clinical skills to complete initial assessments for home care patients in Marin County. You'll conduct Start of Care admissions, follow-ups, and collaborate with an interdisciplinary team. This role offers flexible weekend schedules and a supportive work environment.

🎯 What You'll Do

• Conduct Start of Care (SOC) Admissions using OASIS
• Perform home visits for evaluations and follow-ups
• Initiate and administer IV medications (antibiotics, diuretics)
• Document observations and interventions accurately

📋 Requirements

• Current and unrestricted CA RN license
• Current BLS/CPR certification
• Valid CA Driver's License
• Experience conducting OASIS assessments

• Experience with high acuity patients
• Previous IV medication infusion experience
• Case management fundamentals
