💼 About This Role

You'll join Remedy Home Health Care as a Physical Therapist providing skilled rehab services to senior patients in their homes. You'll help patients regain independence through fall prevention, strength building, and mobility training. This role offers flexible per diem, part-time, or full-time schedules.

🎯 What You'll Do

• Conduct in-home physical therapy evaluations and reassessments
• Develop and implement personalized rehabilitation care plans
• Improve strength, balance, mobility, and functional outcomes
• Document visits using WellSky (Kinnser) EMR

📋 Requirements

• Licensed Physical Therapist (PT) – California
• CPR Certification (AHA approved)
• Minimum 1 year of clinical experience
• Valid driver's license, reliable transportation, and auto insurance

✨ Nice to Have

• Home Health experience preferred
• Experience in senior care, geriatrics, rehab, or community-based care preferred
