💼 About This Role

You'll join the interdisciplinary team at WelbeHealth PACE to develop and implement nutrition services for seniors. You'll conduct assessments, provide counseling, and ensure compliance, making a direct impact on participants' care journeys. This hybrid role offers a $15k sign-on bonus and work-life balance with dedicated PTO.

🎯 What You'll Do

• Conduct nutrition assessments to determine participant needs and goals
• Participate in IDT meetings to develop care plans
• Deliver nutrition interventions and counseling per physician orders
• Oversee menu planning and meal delivery compliance

📋 Requirements

• Registered Dietitian with 2 years directing nutrition services in healthcare
• Current BLS and First Aid certification
• 1 year experience with frail or elderly population
