💼 About This Role

You'll be an integral member of a multi-disciplinary team providing rehabilitative care and skilled nursing to adult and geriatric clients in senior living communities. Your impact includes helping clients remain at home while receiving the medical care they need. This role offers a $1,500 sign-on bonus for home health experience.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements of mobility and function
• Develop and implement individualized care plans with therapeutic exercises
• Educate clients and families on rehabilitative care and activities

📋 Requirements

• Current Pennsylvania Physical Therapist license
• Minimum one year of recent work experience as a Physical Therapist
• Graduation from a CAPTE-approved physical therapy program
