Description

You will provide psychosocial services to terminally ill patients and their families in a home health setting, conducting assessments, counseling, and coordinating care as part of an interdisciplinary team. This role involves local travel in the greater Lebanon, Ohio area to deliver compassionate, personalized care.

Requirements

• Master's degree in social work from a CSWE-accredited program
• Effective communication, comprehension, documentation, and computer skills
• Ability to assess patient needs and formulate individualized care plans
• Ability to work in a field setting with sound judgment

Responsibilities

• Conduct initial comprehensive psychosocial and bereavement risk assessments within 5 days of hospice election
• Facilitate referrals to community resources and financial assistance
• Provide psychosocial and anticipatory grief counseling to patients and families
• Participate in interdisciplinary team meetings and care plan development
• Maintain accurate clinical documentation and assist with discharge planning
