You'll provide hands-on therapeutic interventions in patients' homes as part of a supportive, award-winning team. You'll build clinical skills through follow-up visits with mentorship from a case manager. This entry-level field role focuses on patient care with strong leadership support.

🎯 What You'll Do

• Provide hands-on therapy including gait training and exercises.
• Conduct patient evaluations and implement care plans.
• Document patient care within 24-48 hours.
• Educate patients and families on mobility and safety.

📋 Requirements

• Valid California PT License
• Current BLS/CPR Certification
• Valid CA driver's license with clean driving record
• Graduate of accredited Physical Therapy program
