💼 About This Role

You'll provide home health occupational therapy visits to adult and geriatric patients in the Manayunk, Roxborough, Chestnut Hill, and Mainline areas. As part of a multidisciplinary team, you'll perform evaluations, develop care plans, and implement interventions to help clients remain at home. This role offers autonomy and a supportive team environment with PPE supplies and training opportunities.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations of patients' functional status
• Develop individualized intervention plans to achieve favorable outcomes
• Evaluate home environment for hazards or barriers to independent living
• Accurately document observations and interventions using a touch pad tablet

📋 Requirements

• A current PA Occupational Therapist license
• A minimum of one year of recent work experience as an OT
• Graduation from an accredited occupational therapy program
• Solid computer skills
