🎯 What You'll Do

• Conduct social service assessments upon admission and after hospitalization
• Coordinate care plans with interdisciplinary team and complete documentation
• Assist residents with Medicaid applications, mail, and scheduling appointments
• Address resident and family concerns and educate on Resident Rights

📋 Requirements

• High school diploma or equivalent
• One year experience in long-term care social service department
• Knowledge of geriatric population growth and development
• Ability to perform documentation in writing and electronically
