💼 About This Role

You'll conduct remote assessments for home health patients in SoCal/OC on weekends, acting on Circadia platform alerts to prevent hospitalizations. This role combines telehealth with AI-driven insights for high-risk patient care.

🎯 What You'll Do

• Conduct follow-up and urgent telehealth assessments on flagged patients
• Interpret Circadia platform trends to prioritize high-risk patients and intervene
• Complete documentation same day using AI-powered tools
• Coordinate with on-call providers and care teams for continuity

📋 Requirements

• Certified and licensed Nurse Practitioner in California
• 2+ years NP experience with complex adult populations in home health or post-acute settings
• Eligible for Medicare participation
• Weekend availability (Saturday and/or Sunday)

✨ Nice to Have

• Tech-savvy with Google Suite, multiple EMRs (HCHB preferred), Slack, AI tools
• Spanish language skills
