💼 About This Role

You'll provide hands-on patient care in patients' homes as part of the largest home health agency in San Francisco. Your work will directly reduce hospital readmission rates, with our consistent 10% benchmark outperforming the industry average of 15%.

🎯 What You'll Do

• Perform skilled nursing procedures within RN scope (wound care, tube management, IV therapy).
• Implement and update the plan of care under the Case Management RN.
• Document patient care accurately within 24-48 hours.
• Participate in weekly case conferences and on-call weekend rotation.

📋 Requirements

• 1 year of recent RN or LVN experience.
• Current CA Registered Nurse License.
• Current Basic Life Support (BLS) card.
• Valid CA Driver's License with acceptable driving record.
