You'll provide home health social work services to patients in central Massachusetts, collaborating with a multi-disciplinary care team. Your work enables patients to receive medical care at home. This per diem role offers flexible weekday hours.

🎯 What You'll Do

• Make home visits to clients in designated territories

📋 Requirements

• Master's Degree in Social Work
• Licensed Clinical Social Worker (LCSW) or LICSW in Massachusetts
• Prior home care experience preferred but not required
