Description

You will plan, coordinate, and document skilled nursing care for home health patients, acting as a case manager to develop and implement individualized care plans in compliance with regulations. You'll collaborate with physicians and supervise home health aides while traveling to patients' homes in the greater Cincinnati area.

Requirements

• Valid Ohio Registered Nurse license
• Current CPR certification
• Minimum 1 year clinical nursing experience
• Licensed driver with insured automobile
• Graduate of approved nursing program

Responsibilities

• Complete initial assessment and comprehensive physical history of patients
• Develop and revise plan of care based on nursing diagnosis
• Administer medications and treatments as prescribed
• Counsel patients and families on nursing needs
• Supervise and evaluate home health aide care
