🎯 What You'll Do

• Visit members for in-home health risk assessments.
• Evaluate medical history, review systems, and reconcile medications.
• Perform comprehensive physical exams and point-of-care testing.
• Document diagnoses for care management and treatment planning.

📋 Requirements

• Valid unencumbered Maryland NP license.
• 1-3 years of patient care experience.
• EMR skills preferred (listed as plus, but required for role).
• Strong empathy, compassion, and communication skills.

• Experience with point-of-care testing.
