Description

You will provide rehabilitative care and skilled nursing to adult and geriatric patients in their homes, helping them achieve independence and improved quality of life. As part of a multi-disciplinary team, you'll perform evaluations, develop treatment plans, and educate patients and families.

Requirements

• Current VA Occupational Therapist license
• Minimum one year recent work experience as OT
• Graduation from accredited occupational therapy program
• Ability to work independently and manage time
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Make home visits to clients in designated territories
• Perform thorough evaluations of functional status
• Develop individualized intervention plans
• Assess home environment for hazards and equipment needs
• Educate patients and family members on rehabilitative care
